We shall collect personal/ contact information that is required to provide care, administrate the care that is provided, communicate with clients, and maintain a safe environment for the clients and staffs. Unless the purposes for collecting personal information are obvious and the patient voluntarily provides his or her personal information for those purposes, we will communicate the purposes for which personal information is being collected, either orally or in writing, before or at the time of collection. We shall not collect any other information, or allow information to be used for other purposes, without the patient’s express consent – except where authorized to do so by law. These limits on collection ensure that we do not collect unnecessary information.

2. Client personal health information, which may include

   • Medical history

   • Presenting symptoms, examinations, treatments, lab reports, diagnostic studies

   • Images (Before & After treatments)

3. We collect the following staff personal/ contact information:

   • Name

   • Date of birth

   • Phone Number

   • Residential Address

Part II. Use Of Personal Information

1. Personal information collected from clients is used by this clinic for the purposes of:

   • Identification and contact

   • Provision and continuity of care

     • Historical record

     • Health and beauty promotion and prevention

   • Administrate the care that is provided

     • Prioritization of appointment scheduling

   • Professional requirements

     • Risk or error management

   • Research studies and trials when applicable

   • Processing payment

2. Unless requested by clients, we may contact you via email or phone to inform you about promotion, products or services, or changes to administration/management procedures.

3. Personal information collected from staff is used by this clinic for the purposes of:

REFUND POLICY

At Luminance Health & Beauty Clinic, we are committed to providing exceptional care and service. We strive for excellence in every treatment, and your satisfaction is important to us. However, due to the nature of our services, we have established the following refund policy:

1. Services Rendered

All services provided at our clinic are non-refundable. Once a treatment has been completed, we cannot offer refunds for the service, regardless of the outcome. We encourage open communication with your provider during your consultation and treatment process to ensure your goals and expectations are clearly understood.

2. Prepaid Packages & Treatment Plans

• Prepaid packages are non-refundable but may be transferred or credited to another service or individual with management approval.

• If you choose to discontinue a treatment plan early, the used portion will be recalculated at regular (non-discounted) rates. Any remaining balance may be applied as a clinic credit.

3. Product Returns

• Unopened retail products in original condition may be returned within 7 days of purchase for a clinic credit.

• Opened or used products are not eligible for return due to health and hygiene regulations.

 

4. Missed Appointments & Late Cancellations

• A 24-hour notice is required to reschedule or cancel appointments.

• Failure to provide notice will result in a $50 cancellation fee or forfeiture of a session from a prepaid package.

 

5. Adverse Reactions or Dissatisfaction

If you experience an unexpected reaction or are dissatisfied with your results, we invite you to schedule a follow-up consultation so we can assess your concerns and provide an appropriate solution. While refunds are not issued, we are committed to working with you toward a satisfactory resolution.
