We take privacy very serious and are committed to respecting and protecting your personal information.  This privacy policy details how Luminance Health & Beauty Clinic collects, uses and handles your personal information and how to contact us should you have questions and concerns about our management of your personal information.  Obtaining your consent is crucial to protecting your personal information.  We only handle your personal information in a manner that a reasonable person would consider appropriate in the circumstances.

We shall collect personal/ contact information that is required to provide care, administrate the care that is provided, communicate with clients, and maintain a safe environment for the clients and staffs. Unless the purposes for collecting personal information are obvious and the patient voluntarily provides his or her personal information for those purposes, we will communicate the purposes for which personal information is being collected, either orally or in writing, before or at the time of collection. We shall not collect any other information, or allow information to be used for other purposes, without the patient’s express consent – except where authorized to do so by law. These limits on collection ensure that we do not collect unnecessary information.

  1. We collect the following client personal/ contact information:

    • Name

    • Date of birth

    • Phone Number

    • Residential Address

    • Email Address

  2. Client personal health information, which may include

    • Medical history

    • Presenting symptoms, examinations, treatments, lab reports, diagnostic studies

    • Images (Before & After treatments)

  3. We collect the following staff personal/ contact information:

    • Name

    • Date of birth

    • Phone Number

    • Residential Address

    • Email Address

 

Part II. Use Of Personal Information

  1. Personal information collected from clients is used by this clinic for the purposes of:

    • Identification and contact

    • Provision and continuity of care

      • Historical record

      • Health and beauty promotion and prevention

    • Administrate the care that is provided

      • Prioritization of appointment scheduling

    • Professional requirements

      • Risk or error management

    • Research studies and trials when applicable

    • Processing payment

  2. Unless requested by clients, we may contact you via email or phone to inform you about promotion, products or services, or changes to administration/management procedures.

  3. Personal information collected from staff is used by this clinic for the purposes of:

    • Administrate the employee

    • Processing Wages

RETURN POLICY

Returns for damages or errors made in shipping must be called into our Customer Service within 48 hours of receipt of shipment to obtain a return management authorization (RMA) or have your order corrected.

All other products in unopened, unused retail sizes may be returned within 30 days of initial purchase for a full refund of amount paid, if accompanied by a Return Management Authorization (RMA) number. All shipping charges related to the approved return are the responsibility of the customer. 
 

All return request for qualified goods with a purchase date greater than 30 days of initial purchase may be returned if there is an expiration date of 1 year or more from the date of the RMA request. Returns for goods with a purchase date greater than 30 days will be subject to a restocking fee of 20% of the actual price paid for the products. All shipping costs associated with an approved RMA are the customer’s responsibility.

CANCELATION POLICY

Any cancellations with less than 24 hours of notice are subject to a cancelation fee amounting to the cost of the scheduled service, if the service is part of a package then the package will be debited. Clients who miss their appointments without giving any prior notification will be charged 50% for the scheduled service. Credit Card may be required for future bookings.
