Service Description
Primary care services include preventive care, sick care, and family care. Important Appointment Preparation Notice For your Annual Medical Check-Up/ Preventive Care, please note that fasting blood work is required. To ensure accurate test results: You must fast for at least 8 hours before your appointment. Only water is allowed during the fasting period — no food, coffee, tea, or other beverages. Morning appointments are strongly recommended to minimize discomfort from fasting. If you have any questions or concerns about fasting, please contact our office prior to your visit. We accept most major insurance: Aetna - Blue Cross Blue Shield of Texas - Cigna - Community Health Choice - Humana - Multiplan- Tricare / Humana Military - United Healthcare - Wellpoint
Cancellation Policy
Any cancellations with less than 24 hours of notice are subject to a cancelation fee amounting to the cost of the scheduled service, if the service is part of a package then the package will be debited. Clients who miss their appointments without giving any prior notification will be charged 50% for the scheduled service. Credit Card may be required for future bookings.
